I am an attorney and citizen of Louisiana over the age of 18. Pursuant to applicable law, please allow this letter to serve as a formal request to review and, if deemed necessary at the time of inspection, to obtain copies of the following public records of the City of New Orleans Public Works.

These records would pertain to cleaning catch basins located between 2900 and 4000 blocks of Chartres Street between January 2014 and April 2016.

Time is of the essence. Please direct your custodian of records to allow for this inspection and photocopying of the records detailed on the following pages within (3) days of receipt of this letter as required by law.

Requested Records

  1. Any Dep’t of Public Works records, documents/reports concerning the cleaning of catch basins or related systems located between the 2900 and 4000 blocks of Chartres Street between January 2014 and April 2016.
  2. The names, addresses, and personal phone numbers of all New Orleans Department of Public Works employees, inspectors and managers associated with the cleaning of catch basin systems located between the 2900 and 4000 blocks of Chartres Street between January 2014 and April 2016. As provided under La. Atty. Gen. Op. No. 00-314, this request is directed to the full legal names of Department of Public Works employees, their individual private, and Department of Public Works telephone numbers, and their individual private addresses, unless the employee’s private telephone number is “private or unlisted from a public record”, otherwise, all of their public contacts are requested.
  3. Any written information relating to orders directing the Department of Public Works to clean the catch basin systems located between 2900 and 4000 blocks of Chartres Street between January 2014 and April 2016.
  4. What is the ticket number issued by the maintenance department of the Department of Public Works to any person who reported the inoperative catch basins located between the 2900 and 4000 blocks of Chartres Street between January 2014 and April 2016.

We would like to conduct this inspection on May 17, 2017. Please advise us where the records and custodian will be located.
