For the employee Eric Noble Scott, please provide the following:

Employment Records, Personnel and payroll, employment applications, medical reports, accident reports and any other information or documentation which you may have regarding your employment of the above-named individual at any time. Please also release certified copies of any and all applications, reports, licenses, degrees, certificates, diplomas, test scores, exams, course descriptions, report cards, personal files, correspondence, attendance records and any other documetns you have on claimant. No verbal or written communications, other than the release of medical information and personal file documentation as indicated above, is authorized. This includes any other information you may have concerning any claim for or payment of compensation and/or disability benefits to the above-named individual at any time.


Please note that there is an affidavit for the custodian of records to sign that is scanned into this request.
